Becoming A Patient
Here at 4TrueHealth Functional Center we strive to make your first experience with us as smooth and easy as possible.
The best way to begin the journey towards your True Health is to become familiar with our website. We have done our best to present to you every aspect of our practice in easy to understand terms.
The next step is to make your first appointment either by phone or via our website.
Before your first visit you will have to complete our Informed Consent Form and our Comprehensive Initial Patient Intake Form. We can either mail you a hard copy, email it to your preferred email via secure encryption, or you can download it from our website.
We strongly recommend that you complete and return these forms prior to your first appointment so that we have your consent to be examined and treated, and for Dr. Stauceanu to become familiar with the actual stage of your health and health needs.
After reviewing your Comprehensive Initial Patient Intake Form, Dr. Stauceanu will decide whether it is in your best interest to have an in-office first visit where he can thoroughly examine you, or if a telehealth visit will be sufficient.
We recommend that you wear or bring with you loose clothing or gym attire for your comfort during the thorough physical examination.
